Step 1
Gather the required documents and information. You need the car title and keys. You'll also need the Vehicle Identification Number, which you can find on the title, car registration, and/or your proof of insurance card. In some states, you must have your signature on the title notarized. A few states require you to leave the license plates with the car. Learn the requirements from your state's Department of Motor Vehicles or online through the NKF Kidneycar Donation website. (Follow the first link in Resources.)

Step 2
Fill out a car-donation form. You can call the National Kidney Foundation donation line at (888) 319-2277 and request a form, or fill out the donation form online. You'll need to enter your zip code and the Vehicle Identification Number so the donation will conform to your state's regulations.

Provide your personal information, such as your name, address and telephone number and the year and make of the car. You can also donate most other motorized vehicles and small boats. You will need to describe the condition of the vehicle (such as any noting major body damage and whether or not it is running).

Expect to be contacted within a week. Your state kidney foundation will arrange for a free pickup of your donated vehicle. Be sure you have the car keys ready, and leave the car tags and/or registration with the car if your state requires you to do so. Make sure the title is correctly filled out and signed to transfer ownership.

Step 5
Keep your receipt for your records. Your car will be sold and you'll be sent an acknowledgment listing the sale price within 30 days of the sale. You'll need this to receive your tax deduction. As of 2005, the sale price determines how much you can deduct. (If the sale price is less than $500, you can substitute fair market value up to $499).
