You want to make sure it's properly placed. It will be different on a man than a woman. On a man a little more material will need to be used because the nipples are generally small. It will be about 16th of an inch more into the areola because the nipple is generally tinier and there is not enough to support a piercing. On a woman the piercing is usually done where the nipple itself meets the areola on both sides. Once they are in, they just need to be cared for properly because, especially on a woman, it could be really painful if it gets infected. They rarely get infected. Poor positioning is the main reason we see problems.
