There are different hand positions and meditations for different ailments or illnesses. Also, there are different Reiki hand positions for different situations. The hand positions in Usui Reiki generally concentrate on the seven different chakras of the body. The crown chakra is generally the most common chakra, because it is said to be anywhere on the entire body. The third eye chakra corresponds with the face, nose and eyes and is usually focused on by placing the hands over the patient's forehead.

The throat chakra corresponds with the throat and lungs and is (as you would expect) located over the throat. The heart chakra, with it's equally as assumptive position, is used to treat the heart and the arms. The solar plexus chakra is relative to the stomach, spleen and small intestine as well as the gall bladder and the liver.

The navel chakra belongs to the reproduction system; the testicles and ovaries, respectively. It corresponds with treatment of the bladder and kidneys as well. The root chakra is the last of the seven chakras and is used for the large intestine and rectum. To concentrate on this area, Reiki Master-Teachers place their hands in the area of the adrenal gland.

Cramer advises that there are several different positions. One of the positions she describes is putting "one hand on the base of the person's neck and the other hand on the forehead, and then you imagine sending energies back and forth through the head from one spot to another."

For headaches, there are four different positions that are used to treat patients. Allergy headaches, as well as headaches associated with colds or the flu, are treated with the first hand position. This position has the practitioners' fingers covering the eyes, with their palms resting on the forehead. Treating yourself is much the same, only your palms are covering your eyes, and your fingers are resting on your forehead.

Stress headaches usually occur in the temporal lobes, or the front of the head. Placing your fingers over the temples and your palms on the upper part of the head is position number two. If you are treating yourself, place your palms over your temples and your fingers resting on the top of your head.

Other headaches, sometimes associated with the ears can be treated with a third hand position. The practitioner covers the ears with their palms with a "cupping" position. This is also good for headaches associated with a cold, the flu, ear infections, or hearing problems. Self treatment involves cupping your own ears with your fingers pointed towards the top of your head.

The fourth hand position is used to cure another kind of stress headache. Headaches often originate at the base of the neck, or the nape. By placing their hands underneath the head, the practitioners can relieve pressure and treat headaches, neck and back problems. Treating yourself is much the same, with your palms cupping the back of your head; fingers are pointed to the top of your head.
